My name is Katie and I am your local Community Connector for a new social prescribing service called Connected Communities. Connected Communities works in various areas of rural east Suffolk. Our role is to work with anyone over the age of 65 in these areas to consider what matters to them and link them to local services which could improve their health and wellbeing. Our primary focus is on supporting those who are isolated or lonely however we will work to support those with a range of life issues. We use health coaching conversations to get to know the individual and find out what matters to them and what they would like to change to make them feel more fulfilled and happier. We then come up with a plan with them and link them to the right support. Ordinarily we would meet face to face but at this time we are offering telephone support and assessing if socially distance meet ups would be appropriate. We also have our very own mobile home called “Vincent Van Go” Vincent travels around rural areas so we can get to those who really need us but also offers a safe space to talk. We are currently looking at covid safe ways of utilizing “Vincent Van Go”.
Attached is our leaflet and a referral form. People can self-refer to us or a third party can make a referral with their permission. You can either refer by calling us on 01473 835477 or emailing Connectedcommunities@suffolkfamilycarers.org You can also complete the attached form and either send to us by post Connected Communities, Unit 9, Hill View Business Park, Old Ipswich Road, Claydon, IP6 0AJ. If you wish to email the form to us securely simply attached the form and send to our email address but adding [encrypt] including the square brackets in the subject line.

Nurse Daisy Faddy who lives in Ubbeston has indicated that she is willing to try and help anyone in our parish who might need assistance with medical issues. From a chat on the phone to collecting medication or guiding people in their quest to find specific answers, she will do her best to help.
